Westchester Jewish Community Services Care Coordinator, Children's Health Homes in Yonkers, New York

Details about this exciting opportunity:

  • Full-Time Care Coordinator to work as a part of our Children’s Health Homes program.

  • Compensation: $50k-$55k/year.

  • The Care Coordinator will work with youth who struggle with mental health needs and/or medical needs that impact their ability to function successfully at home, school, and the community.

    Responsibilities include (but are not limited to):

  • Provide care coordination services to youth/adolescents.

  • Providing crisis intervention and prevention services as necessary.

  • Maintain records to conform to agency policies and regulatory agencies directives.

  • Working closely with a variety of providers and family members to ensure the youth’s success and positive mental wellness.

    If you have the following:

    Qualifications:

  • Bachelor’s degree with two years of experience working with children/adolescents required, Master’s degree with one year of experience working with children/adolescents preferred.

  • Driver’s license and a vehicle are required.

    And Attributes:

  • Ability to work with youth/adolescents/young adults from 0-21 years old.

  • Ability to engage and work with parents/caregivers.

  • Good advocacy skills.
