We are currently hiring a Care Coordinator – Schenectady Office, to work in our Care Coordination Services. As the Care Coordinator, you would be responsible for:

  • Obtain written participant consent via engagement through progressive outreach such as written correspondence, phone calls, home visits and collaboration with other providers.

  • Develop a care plan through case conferencing/consultation with all member’s providers, to guide all service delivery and achievement of desired program outcomes.

  • Conduct home visits, as appropriate, and monitor member health status, access to services and quality of received services.

  • Maintain case records and documentation to meet NYSDOH, lead Health Home and agency standards.

Care Coordinator Minimum Salary: $19.23

Care Coordinator Maximum Salary: $21.02

Care Coordinator Schedule: Monday – Friday 8:30AM to 4:30PM

Qualifications:

  • 4 year/Bachelor’s Degree List field of study: in public health, human services or related field.

  • Preferred Licensed level clinician with prior experience in a behavioral health clinician or care management supervisory capacity; Working knowledge of the NYS Health Home Program.

  • NYS licensure and current registration as a Registered Nurse.
