Description

Under general direction, provides intergenerational family intake, assessment, information, consultation and referral, data collection, analysis, and reporting for the NYS Kinship Navigator Program. Provides virtual case management for defined region in NYS. Assists with event planning, education, and policy work.

Starting Rate: $22.00 / hour

Essential Duties and Responsibilities

  • Provides intake, assessment and consultation to kinship caregivers.

  • Responds to consumer calls using up-to-date knowledge of resources available to kinship caregivers across New York State. Appropriately responds to requests for information and referrals

  • Uses social work skills to provide telephone assessment and consultation on kinship care issues

  • Identifies request requiring immediate assistance (i.e. safety issues or emergent basic needs such as food, shelter or medical care)

  • Identifies requests requiring further assistance and advocacy

  • Conducts legal telephone follow-ups; provides random call backs for evaluative purposes as per program requirements as needed

  • Uses computer software to track all calls, intakes and client outcomes

  • Identifies callers who need a more extensive evaluation conducted by Director

  • Maintains timely and accurate documentation of services provided

  • Completes mailing of information and resources to caregivers

  • Provides case management and assistance to caregivers in designated region of New York State

  • Participate in all relevant actions/tasks and support activities related to the Kinship Navigator’s federal project as outlined in the work plan submitted to the Office of Children and Family Services and attached herein

  • Develop all related skills and knowledge related to the project and to the Kinship Navigator program deliverables

  • Participate in Kinship Navigator related planning and implementation discussions

  • Help to establish and operate all program elements related to supporting program participants, including:

  • Increase efforts to contact caregivers and professionals

  • Assist in implementation of the Permission to Contact procedure

  • Complete intakes and data collection for caregivers in catchment;

  • Increase program participant’s knowledge of available community resources;

  • Increase participant’s knowledge of financial assistance grants;

  • Increase participant’s connections to parent trainings and coping skills;

  • Increasing participant’s linkages to trauma informed care such as mental health evaluations, substance abuse resources;

  • Assist with regular follow up calls to caregivers;

  • Assist in planning for sustainability;

  • Provide monthly reports on performance and other documentation related to the project;

  • Perform any and all other actions that support the contractor’s performance of his/her responsibilities.

  • Create and maintain relationships with local kinship programs in geographic catchment

  • Create and maintain relationships with local agency staff (DSS, courts, county offices, etc.)

  • Presents at regional conferences/events

  • Fosters and builds relationships/networks both inside and outside the organization

  • Assists in other funded projects as needed

  • Monitors and updates information, performs surveys and other research projects.

  • Maintains up-to-date professional knowledge through reading, workshops, and conferences

  • Participates as a member of the Kinship Navigator program in order to facilitate the delivery of services.

  • Is aware of and actively supports the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.

  • Other duties as requested by Supervisor.

Qualifications

Education: Bachelor’s Degree in a related field preferred. Equivalent Combination of education and experience will be considered.

Credentials: N/A

Experience: Minimum oftwo years of human services experience. Customer service experience in a non-profit organization is also preferred.
