Apicha Community Health Center DIRECTOR OF POPULATION HEALTH MANAGEMENT in New York, New York

Position Summary:

We're looking for a committed, professional full-time Director of Population Health for our Manhattan location. The ideal candidate is someone who is a ccountable for the design and implementation of Apicha’s population health strategy. This position will be responsible for understanding population health payment models, aligning programs to achieve specific outcomes, continuous monitoring, and process improvement. Responsible for the prioritization and implementation of activities relating to clinical and operational workflow, quality improvement, data quality and analytics, UDS reporting, and data strategy development. The Director’s portfolio will include, but not be limited to, transitions-in-care, social drivers of health, HEDIS/risk adjustment reporting, preferred partner relationships, and patient engagement strategy.

Job Responsibilities:

  • Population Health - develops, monitors, and educates on key metrics and programs to support improved care and cost outcomes.

  • Quality Improvement and Quality Assurance - develops and implements the QI/QA Plan in collaboration with the Medical Director.

  • Health Informatics - oversees the Health Informatics activities for the organization and leads data standardization and quality initiatives, ensuring integration of disparate data sources.

    Qualifications:

  • Advanced degree in nursing, health related field, health care administration, business administration, or public administration required; RN or other clinical background preferred.

  • 3+ years of experience working in a physician practice, health care services, population health, insurance or related setting required.

  • Experience with community health centers/Federally Qualified Health Centers (FQHCs), a plus.

  • Knowledge of population health, quality of care concepts and methodologies, as well as concepts of community health and social determinants of health.

  • Experience working with Healthcare Effectiveness Data and Information Set (HEDIS) measures and instituting appropriate interventions required.

  • Knowledge of clinical guidelines related to PCMH preferred.

  • Knowledge of value-based contracts preferred.

  • Proficiency in the use of Microsoft Office (Word, Excel, PowerPoint and Outlook) required.
