HAPCAP Health Equity and Outreach Coordinator in Logan, Ohio

We are excited to announce an immediate opening for a Health Equity and Outreach Coordinator with our HAPCAP for Health team at the Southeast Ohio Foodbank in Logan, OH.  As a member of our team, you will work to further health access, food security, and equity for our rural neighbors. 

The coordinator will oversee grant applications, financials, and overall operations for HAPCAP's health equity programs. These programs include Bringing Healthy Home, which offers wraparound services to pregnant women and their families who are facing food insecurity, and FoodRx, which utilizes referrals from healthcare partners to connect neighbors to local food resources. The Coordinator will supervise the Hunger and Health Manager and a team of Community Health Workers.

The Health Equity and Outreach Coordinator will be a liaison with community organizations, healthcare systems, funders, and Medicaid providers to deliver valuable health equity programming to vulnerable neighbors. This includes identifying new partners in order to expand programming to fill and bridge gaps in services. As such, successful candidates will have the ability to work well with a wide range of staff, partners, neighbors, and vendors.

Good leadership, relationship, organizational, and management skills are required. The Coordinator will ensure program compliance according to grant, state, and national regulations, as well as to submit required reports to funders. Therefore, some experience managing grants is preferred, though not required. 

A commitment to addressing the causes and conditions of poverty and hunger is essential. A high school diploma or GED is required, but a college degree is preferred. Prior experience in foodbanking, health equity, and/or antipoverty work is preferred, but not required. CHW certification is preferred, but not required. The candidate will be required to complete a state-recognized CHW certification course within a year of being hired.

A valid driver's license and background check are required.

This is a full-time position within a dynamic, service-oriented work environment. The salary range for this position is $46,000-$52,000 yearly, commensurate with experience, along with 100% health, dental, and vision coverage for the employee, a generous paid time off package, retirement plan with 6% employer matching, and periodic Cost of Living Adjustments (COLA).
