The Harm Reduction Mobile Outreach Coordinator oversees the operation of Evergreen Health’s Mobile Unit which includes: Outreach to people who use drugs (PWUD) in order to engage them in services as appropriate, collaboration with community stakeholders and partners to expand outreach services to new locations and underserved areas, involvement in Peer Delivered Syringe Exchange services, and delivery of Opioid Overdose Prevention and other harm reduction program services in order to properly fulfill performance outcomes in relation to mobile services. The Harm Reduction Mobile Outreach Coordinator conducts administrative and direct service functions to support the day-to-day operations of the program.

As part of the essential functions of this role, the Harm Reduction Mobile Outreach Coordinator:

  • Implements exchange work plans, goals and objectives targeting special populations that include: PWID, gay and bisexual men, youth and people of color.

  • Coordinates and leads enhanced outreach for SEP and other harm reduction programs in high risk populations and communities using identified strategies; outreach sessions may make use of SEP Mobile Unit, or not, depending on the situation.

  • Conducts individual harm reduction sessions with high risk clients; makes referrals based on identified need; follows up on referrals made.

  • Links clients to supportive, behavioral, community, medical and other services as needed and follows up on referrals made.

  • Works with various internal and external programs to ensure continuity of care and successful navigation and retention in services for patients/participants.

  • Assists in tracking of data and preparation of monthly and annual reports of activity to New York State Department of Health and the Centers for Disease Control.

  • Effectively communicates with their supervisor to ensure the integrity and consistency of services offered through Mobile Outreach service; properly adheres to established safety protocols and other SEP policies and procedures

  • Works with their supervisor to promote and manage NYS DOH waivered mobile exchange sites, Peer Delivered Syringe Exchange services, and Special Arrangements, as needed.

  • Provides continuous Opioid Overdose Prevention Training to Evergreen Health participants, peers, and other external communities.

    Qualified candidate will have a Bachelor’s degree and two (2) years of experience providing services directly to clients in a healthcare/prevention program setting—OR—two years of being actively engaged in managing a prevention/health program—OR—a combination of education and experience. Candidate must have the ability to establish rapport with PWUD and provide services to this population in a context of empathy and non-judgement. Knowledge of drug use and substance use disorders, and of systemic issues faced by PWUD. Familiarity with computers and MS Office software (Outlook, Word) preferred. Possess valid NYS driver license and insured, dependable car to use for traveling to offsite locations to conduct business on behalf of the Association. Sensitivity toward cultural diversity, people living with HIV/AIDS and lifestyle and addiction issues essential.

    Required education: Bachelor's degree

    Required experience: Providing services directly to patients in healthcare/prevention program (2 years) OR managing a prevention health program (2 years)

    Additional requirements: Possess valid NYS driver license and insured, dependable car to use for traveling to offsite locations to conduct business on behalf of the Association
